Leicester City Clinical Commissioning Group (CCG) is committed to improving care for patients and their families and carers who are reaching the end of their life.
End of life care is defined as people who are in the last 12 months of life.
Caring for someone at the end of their life is a sensitive and often difficult subject to discuss, and we need you to help us gather the knowledge which will allow us to understand whether we have the right services in the city for people at the end of their life.
Therefore over the next couple of months we are looking for people to come forward and talk to us about their experience.
